Upon officer's arrival several concerned citizens pointed out broken glass from the apartment in question. Officers arrived at the apartment, knocked, and announced their presence. After hearing a female voice they believed to be in distress, and fearing serious bodily harm was occurring, Officer Hughes kicked the entry door and forced it open. A male subject received a laceration above his left eye as a result. He was transported to Saint Mary's Hospital for treatment. A thorough investigation revealed no crime had occurred and no arrests were made. The entry door suffered minor damage and was still functional. Photographs were taken of the door, the apartment, and the male subject struck with the door. I responded to the scene and notified WC3 Lieutenant Baum of the incident. COMMENTS: The male subject's laceration was caused by the door striking him in the head when Officer Hugh's forced entry (I concur with Sergeant Scaccia the officer had exigency to force entry). I then directed Sergeant Scaccia to file a supervisor's UOF report because of the subject's injury even though the force used was not intentional. For further, see Blue Team UOF report.
