directly in front of Avenue. The entrance of the apartment building faces north. The entrance
to the apartment courtyard had a black steel gate, with the doors open. The courtyard had three sets of stairs.
One set of stairs at the south end of the courtyard, one set that of stairs on the east side of the building and
another set of stairs on the west side of the building.
When I reached the front of the apartment entrance, I saw Officer Cornell and Officer Casillas in the front of the
east/west stairwells. The officers had a subject on the ground handcuffed. The subject (later identified as
Defendant Kyle Blauvelt) was male white, covered in what appeared to be blood. He was wearing light blue jeans,
no shirt and no shoes.
For further, see use of force reports filed by Officer Casillas, Officer Cornell, Officer Arias and Officer Hodgson.
It should be noted that Officer Casillas suffered left knee pain and Officer Cornell suffered right knee pain. These
injuries were sustained due to Blauvelt violently resisting being taken into custody. Both officers were treated at
Occupational Health and released after treatment.
Officer Manusselis took pictures of the use of force scene. For further, seepictures.
I asked the Officer Cornell if they needed assistance, and he told me to go check on the female victim who was
located in the upstairs apartment #9.
I took the east stairwell and entered the apartment at the top of the stairs. The door was open to a living space.
The apartment is a small 1 bedroom and 1-bathroom apartment. On the northwest side of the apartment was a
door that led to a small balcony that faced to the north and out on to Broadway Avenue. On the east side of the
apartment was a small kitchen area. There was a small hallway that led to a bedroom on the southeast side of the
apartment and a bathroom located at the end of the hallway.
When I entered the apartment I immediately noticed large amounts of blood that was on the floor. There were
bloody handprints on the entrance door, drops of blood and footprints of blood near the door to the balcony.
In the living area, there was a brown couch that was against the east wall. On the couch was female (later
identified as Victim wearing dark clothing. was laying on her back, feet facing west and
head her facing east bound. She was receiving medical attention from Paramedics. seemed to be
nonresponsive, her face was extremely swollen, and was covered in blood. She appeared to have major head and
facial trauma.
The balcony glass door was broken and there were bloody handprints on the door near the door handle. On the
balcony floor there was a large amount of blood. There was a brown sheet that was hanging across the balcony
rail. On the floor of the balcony I observed what appeared to be a broken clay pot, glass ashtray, black purse, and
two white iron objects. All of the objects were covered in blood.
Paramedics Rescue #10 transported to St. Mary's Medical Center. Dr.assessed the victim
and her injuries. suffered from 80 stitches, broken cheek bones, loss of consciousness, major swelling, and
bruising. Due to the nature of her injuries, had to be admitted and later transported to the Intensive Care
Unit (ICU).
I collected the glass ashtray, broken clay pot pieces, and the white iron objects; I kept them in my possession until
later placing them into evidence. The items collected are believed to be objects the Blauvelt used to assault
The black purse was later returned to the victim at the hospital.
I located the identification card in the black purse on the balcony.
I contacted the calling party, (later identified as who is an off-duty Los Angeles Police
Department officer (LAPD). told me the following:
was walking on the north side of Broadway Avenue, heading westbound. said she heard the
sound of someone yelling. She looked up and observed a male white yelling obscenities and holding a blunt
object in his hands. identified the subject that was yelling and assaulting the person as the same male
subject that was removed from the apartment by police personnel (Kyle Blauvelt). Blauvelt was using the object to

strike downward at something on the balcony. She could not see what Blauvelt was striking because the balcony
was completely covered by a brown sheet. Blauvelt stopped striking the down and moved the sheet back off the
balcony rails.
Once Blauvelt moved the sheet, said she saw a hand appear from the bottom of the balcony rails.
immediately called the police after she said Blauvelt began to hit something who she believed to be a
person on the balcony once again. After striking the person on the balcony again, Blauvelt, dropped the round
object on the floor of the balcony and proceeded to move the person. The door to the balcony leading in to the
living area was open. Blauvelt placed his forearms underneath the person's armpits and dragged the person back
inside of the apartment. By the time Blauvelt reentered the apartment, responding units had arrived.
I responded to St Mary's Medical Center, and contacted Victim She advised me the following:
arrived home from the grocery store planning to cook dinner for her boyfriend, Kyle Blauvelt, to celebrate
receiving a paycheck today. According to Blauvelt had been irritable because he had not taken his anxiety
medication in the last 8 days.
When she entered the apartment, Blauvelt was in the bedroom reading. While was cooking dinner, Blauvelt
began to throw away the food that she was cooking on the stove. She said Blauvelt threw the food in the trash
three times for no reason.
She said she went to sit on the couch and Blauvelt began yelling at her. Blauvelt got angry and got in her face.
She said that Blauvelt was accusing her of both doing drugs and cheating on him. said she told Blauvelt to
get out of her face and to stop accusing her of things. Blauvelt then punched her in the face. She does not
remember what hand Blauvelt punched her with. believes that Blauvelt knocked her out because the next
thing she remembers is waking up to Paramedics speaking to her. does not remember being hit by any
blunt objects. The last time remembers being on the balcony was earlier today to smoke a cigarette.
Due to the severity of the injuries, I contacted the on call DART Detective (J. Conrad #6156) to advise him of the
incident. He later responded to St. Mary's Medical Center to further the investigation.
Officer Reyes took pictures of injuries and of the apartment. Officer Reyes later uploaded the pictures to
the Mideo database.
I provided with a Report Receipt, and information regarding a Temporary Restraining Order.
I completed an Emergency Protective Order (EPO) issued by Judge Carol J. Najera. For further, see Case
#
I provided with a copy of the EPO at St Mary's Medical Center. I served Blauvelt the EPO at Community
Hospital, 1720 Termino Aveune, Long Beach, CA 90804. I submitted the original copy of the EPO to our records
department.
1S15 (Sergeant S. Hall #5933) was notified of the incident and responded to the scene, due to nature of the
violent crime. She later notified the on-call violent detective cue to the officer injuries.
Officer Arias stood by for a board-up of the front door of the apartment.
Officer Maes collected the clothing was wearing during the time of the incident. Officer Maes kept the items
in her possession until later placing them into evidence. For further, see property report.
Blauvelt was arrested and transported to Community Hospital by a Rescue #4. Blauvelt was later treated and
transported to the downtown booking station by Officer Foley. Officer Foley collected the pants that Blauvelt was
wearing the time of the incident and he kept this item in his possession until later placing it into evidence. For
further, see Officer Foley's follow up arrest report.

Narrative
BWC evidence recorded and reviewed prior to filing the Use-Of-Force. Initial arrival was not covered because the
device didn't activate with my vehicle emergency lights. BWC activated during the UOF.
While working uniformed patrol as unit 1C8 I, Officer Hodgson #5611, assisted units that were en route to a
possible Assault with a Deadly Weapon where the victim was down on the ground.
Call# 1181 read: "CHK FOR POSS 245 W/BRICK ,,, J/O, ,,, SUSP IS A M/W , 20-30, BRO HAIR SHOULDER ,
WRG NO SHRT ,,, VICT IS ON THE BALCONY 929 ,,,".
Unit 1C68 (Officer Cornell #10328 and Officer Casillas #10537) were dispatched and I heard them arrive at the
location and state that there was a female victim on a balcony who was down and not responding, over the Police
radio. Upon my arrival I parked my unit in the #2 eastbound lane of Broadway and saw the Officers of unit 1C68
giving loud (lawful) commands to a male white, later identified as ARR Blauvelt, on a second floor balcony facing
the street. ARR Blauvelt was sitting in the doorway to an apartment on the balcony holding in front of him an
unresponsive female who's face was covered in blood.
Officers were yelling for ARR Blauvelt to release the victim and open the lower gate so we could render aide. ARR
Blauvelt refused to move away from the victim, who remain unresponsive. I requested LBFD respond to our
location with a ladder because I felt that we were going to have to rescue the victim by climbing up to the balcony.
We noticed an emergency key box and requested the supervisor respond (code three) with the access key for the
box.
Officer Casillas requested I get the 40mm less-than-lethal unit from my vehicle and I retrieved it with a bag of
projectiles. I loaded one 40mm "eXact iMpact" projectile into the weapon (see evidence) and returned to the front
of the location in time to see the victim was now being held upright by ARR Blauvelt in front of him. ARR Blauvelt
kept the victim in front of him as he backed into the apartment out of our sight. Someone requested a unit respond
code three with a pick and ram to gain access though the buildings locked front gate.
As we were waiting for the units to arrive the same male from the balcony (ARR Blauvelt) came downstairs and
opened the front access gate. ARR Blauvelt was bare chested and covered in blood. Officer Casillas and Officer
Cornell immediately entered through the open gate and attempted to detain ARR Blauvelt, I followed behind

1C68 with the 40mm. Officer Cornell took control of ARR Blauvelt's right wrist and ARR Blauvelt immediately spun
around to violently punch Officer Casillas twice in the face with his left fist.
I felt that there was an immediacy to subdue and detain ARR Blauvelt so we could render aid to the victim, since
ARR Blauvelt was between us and the stairs to the apartment, where the victim was located. Even though I was
within 6' of ARR Blauvelt I fired the 40mm at his lower leg but ARR Blauvelt was moving and was struck in the
lower back. Due to the sudden violent resistance of ARR Blauvelt I didn't have time to utter a warning. The impact
of the projectile caused ARR Blauvelt to buckle to his knees and Officer Casillas was able to push Blauvelt to the
ground with Officer Cornell still controlling ARR Blauvelt's right wrist. ARR Blauvelt continued to violently resist us
by kicking, screaming, twisting, and striking out at us with his free left hand.
As we were fighting with ARR Blauvelt, in the entry way, personnel from the LBFD arrived and we told them to
respond immediately to the upper apartment so that aide could be rendered to the victim.
Officer Casillas was able to roll ARR Blauvelt onto his stomach and I sat on the back of ARR Blauvelt's hips to
prevent his kicking or rolling back over and to assist with the handcuffing. Officer Cornell was not able to bring
ARR Blauvelt's arm to the rear to be handcuffed so I deployed my Electric Conductive Weapon (ECW). As there
was no room for a proper spread of the probes I removed the cartridge and used the ECW in a "Drive Stun" mode.
I performed "Drive Stun's" to ARR Blauvelt's upper right shoulder blade area and commanded him to "stop
resisting" and to place his hand behind his back. Each time ARR Blauvelt failed to comply with my lawful
command I performed a "Drive Stun". I continued this process 6-8 times before Officer Cornell was able to bring
ARR Blauvelt's hand far enough to the rear so I could place a handcuff on his right wrist. I maintained control of
the handcuffed wrist as Officer Cornell reached across Officer Casillas to take control of ARR Blauvelt's left wrist.
Officer Cornell was able to bring ARR Blauvelt's left wrist to the rear and I placed it into a handcuff. Once ARR
Blauvelt was detained all force stopped and he was placed in a recovery position because Officer Casillas
informed us that he used a carotid restraint on ARR Blauvelt, and he had passed out, enabling us to complete the
handcuffing. I placed a hobble around both of ARR Blauvelt's ankles to prevent him from kicking us. We requested
an additional paramedic unit to transport ARR Blauvelt to the hospital to be medically cleared for booking.
For further on the victims status see the main report.
I requested a Sergeant for the use-of-force and SGT Hall responded.
I retrieved the spent cartridge out of the 40mm weapon and retained it in my possession until placing it into
evidence. I looked for the foam projectile but was unable to locate it for evidence.
Rescue #4 transported ARR Blauvelt to Community Hospital and I followed them with Officer Casillas in the rear
of their unit.
I was later relieved at the hospital and filed this report.

On 12/29/2017 at 1652 hours, my partner (Officer E. Cornell #10328) and I (Officer J. Casillas #10537) were
working uniformed patrol in a marked black and white Police vehicle as Unit 1C68. We were wearing full Long
Beach Police Uniform with LBPD patches on both shoulders and a gold LBPD badge upon my left chest. We were
dispatched to regarding an assault with a deadly weapon. The call stated the victim was
assaulted with a brick and was down on the balcony. For further see call #1181.
Upon arrival, I observed a male subject (later identified as DEFT- Kyle Blauvelt) standing on the 2nd floor
Broadway facing balcony of Kyle had no shirt on and had what appeared to be blood all over
his torso. At his waist, was a female subject (later identified as VIC-who was sitting on the floor
hunched forward. I observed had sustained massive trauma, bleeding and swelling, to her face.
The apartment complex was locked and secure and we were unable to gain entry.
Unit 1C8 (Officer G. Hodgson #5611) assisted at the scene and retrieved a 40mm less lethal launcher from his
Police vehicle.
My partner and I gave Kyle multiple verbal orders to allow us in the complex as he stood on the balcony with
Kyle appeared hesitant and stated he couldn't open the door. Kyle then began to drag
unresponsive body back into the apartment.
I believed needed immediate medical aide and Kyle was delaying our response.
Due to the exigency, Officer Cornell requested a Police unit to respond to the location with pick and ram to
forcefully gain immediate entry into the apartment complex.
As additional Officers were enroute to assist, Kyle, still covered in blood, exited his apartment and opened the front
entry gate.
Officer Cornell immediately entered the apartment's entry corridor and attempted to detain Kyle by reaching for his
left arm. I observed Kyle pull his left arm away from Officer Cornell, step back and postion himself in a fighting
stance by bending his knees, bend forward at the waist and put his arms up infront of his torso. As I approached
Kyle, Kyle immediately struck me one time with a closed right fist to my face. The strike caused me to take a step
back. I immediately heard a loud bang, which was later known to be the discharge of Officer Hodgson's 40mm
less lethal launcher. Kyle, still standing, leaned forward at the waist as Officer Cornell maintained control of his
left arm. I placed both my hands on Kyle's shoulders and pulled him to the ground and placed him onto his

stomach.
Kyle continued to scream and violently kick and pull away from Officers. I gave Kyle numerous commands to stop,
I placed my body weight on Kyle's upper torso and encircled my right arm around Kyle's neck to attain a carotid
control hold. I heard numerous deployments of a Conducted Electrical Weapon as Kyle continued to violently
resist. I began to apply pressure with the carotid control hold when Kyle punched me a second time with a closed
right fist to my face.
The carotid control hold successfully rendered Kyle unconscious. I immediately released the hold which allowed
Officers to place Kyle in handcuffs. Kyle was rolled to his side in the recovery position and I observed Kyle was
breathing and began to regain consciousness.
LBFD was immediately requested to treat Kyle's injuries.
For further on see main and additional follow-up reports.
Unit 1S15 (SGT S. Hall #5933) was notified of the use of the carotid control hold.
I rode with Kyle in LBFD Rescue 4 as he was transported to Community Hospital for treatment. I notified staff of
LBFD and Community Hospital that the carotid control hold was applied.
I sustained a minor injury to my left knee and face caused during the use of force. I was treated and released from
Care On Site Medical.

from the center of the complex. The apartment has a balcony on the north side of the building that can be viewed
from
Upon arrival, Officer Casillas and I observed a subject (later identified as ARR Kyle Blauvelt), sitting on the floor of
the balcony, holding an unconscious female (later identified as VIC I immediately observed
that VIC had sustained significant injuries to her face, and had blood and bruising all over her head, neck
and hands. I also observed ARR Blauvelt was only wearing pants, and had blood smeared on his arms, hands,
chest, abdomen, and pants.
Officer Casillas attempted to open the gate to the complex, but was unable to because it was locked. I ordered
ARR Blauvelt to open the complex gate so we could render aid to VIC ARR Blauvelt told me he could not
open the door, and that VIC needed help.
It should be noted that Officer Casillas and I were wearing our standard issue Long Beach Police patrol uniforms
with patches on our shoulders that read,"Long Beach Police," and a badge on our left chests that reads, "Police
Officer."
I continued to order ARR Blauvelt to open the complex gate for us to render aid. ARR Blauvelt grabbed VIC
around her waist, and tried to drag her back into the apartment. VIC still appeared unconscious,

and unable to fight off ARR Blauvelt. ARR Blauvelt also began yelling at me,but I could not understand what he
was saying.
ARR Blauvelt then dropped VIC on the balcony floor, and went inside the apartment. I continued to order
ARR Blauvelt to open the complex door, but received no response. After approximately one minute, ARR Blauvelt
returned to the balcony, grabbed VIC around her waist, and stood her up, facing Broadway, against the
balcony guard rails. I continued to order ARR Blauvelt to open the complex gate, but he did not respond to my
commands.
ARR Blauvelt then dragged VIC into the apartment, out of sight. I feared that ARR Blauvelt would continue
to inflict great bodily harm on VIC I aired via my police radio for Unit 1S15 (Sgt. S. Hall # 5933) to
respond with, "Pick," and "Ram," devices to force open the complex door and, if necessary, the apartment door.
Units 1C8 (Officer G. Hodgson #5611) and 1C7 (Officer G. Arias #5520) also assisted on the call, and responded
to the scene. It should be noted that Officer Hodgson deployed a 40 MM Less Lethal launcher that he obtained
from his patrol vehicle.
After ARR Blauvelt dragged VIC inside of the apartment, he walked down the stairwell leading from the
apartment into the center of the complex. I ordered ARR Blauvelt to open the complex gate door, and he finally
complied. After opening the complex door, ARR Blauvelt began to walk back towards the stairwell of the
apartment.
I feared that ARR Blauvelt was going to go back into the apartment to inflict more injuries to VIC For
officer safety, I decided to detain ARR Blauvelt, pending further investigation. While ARR Blauvelt was still on the
ground floor of the complex, I attempted to place a rear wrist lock on ARR Blauvelt's left wrist with both of my
hands.
Officer Casillas also tried to assist me in placing a rear wrist lock on ARR Blauvelt. As I placed both of my hands
on ARR Blauvelt's left wrist, I heard him yell something I did not understand, then pull away violently from my hold.
I then saw ARR Blauvelt punch Officer Casillas one time in the center of his face with a right fist.
Officer Casillas and I attempted to regain control of ARR Blauvelt's arms in order to place him in handcuffs. I
attempted to gain control of ARR Blauvelt's left arm, and Officer Casillas attempted to gain control of ARR
Blauvelt's right arm. ARR Blauvelt continued to violently thrash his arms in an attempt to escape from our hold.
Officer Hodgson, who was standing at the complex fired a single 40 MM less lethal round from the launcher he
deployed, striking ARR Blauvelt in the lower portion of his back.
ARR Blauvelt was struck with the 40 MM less lethal round, causing him to bend forward at his waist. Officer
Casillas then placed his hands on each of ARR Blauvelt's shoulders, and applied downward pressure in order to
take ARR Blauvelt to the ground and handcuff him. I also applied downward pressure on ARR Blauvelt's left arm.
ARR Blauvelt spun around to wear his head was facing northbound, and went to the ground. I also went to the
ground, and struck my right knee on the floor.
While on the ground, ARR Blauvelt continued to violently resist by kicking his legs and thrashing his arms. I
attempted to gain control of ARR Blauvelt's right arm, but he continued to thrash it, and punched my right elbow
with his right fist. Because ARR Blauvelt continued to violently resist being taken into custody, Officer Casillas
performed a carotid restraint hold.
As Officer Casillas performed the carotid restraint, ARR Blauvelt continued to thrash his body, kick his legs, and
thrash his arms. Officer Hodgson removed the cartridge of his Conducted Electrical weapon, and drive stunned
ARR Blauvelt on his lower back. It should be noted that Officer Hodgson continued to yell, "Stop Resisting!" at
ARR Blauvelt.
After Officer Hodgson drive stunned ARR Blauvelt's right bicep, I was able to place ARR Blauvelt's right arm in a
rear wrist lock. Officer Hodgson was able to place a handcuff on ARR Blauvelt's right wrist, and took control of the
arm. I reached over Officer Casillas, and gained control of ARR Blauvelt's left arm. I placed ARR Blauvelt's left
arm behind his back, and Officer Hodgson placed the other handcuff on ARR Blauvelt's left wrist.

Narrative
After handcuffs were placed on ARR Blauvelt, Officer Casillas released his carotid restraint hold, and I placed
ARR Blauvelt in the recovery position. ARR Blauvelt continued to violently thrash his legs. Because ARR
Blauvelt continued to thrash his legs, Officer Hodgson placed a hobble restraint over ARR Blauvelt's ankles.
After ARR Blauvelt was taken into custody, I immediately aired for a second paramedic unit to respond to the
scene and evaluate ARR Blauvelt. Long Beach Fire Department Rescue 4 responded to the scene and evaluated
ARR Blauvelt. Rescue 4 transported ARR Blauvelt to Community Hospital Long Beach for booking clearance.
While in the Emergency Room of Community Hospital, I read ARR Blauvelt his Miranda Rights per form
PD1000.008, to which he replied "Yes" to waiver question #1. I asked ARR Blauvelt about the incident, and he
stated the following: ARR Blauvelt stated that he and VIC have been in a relationship for 7 years, they
have lived together for 7 years, 4 years have been in their current residence, and they have no children in
common.
ARR Blauvelt further stated that for the last four days, he and VIC have been "Stressed Out," and that he
believed today was the end of the world. I asked ARR Blauvelt what the incident was about, and he stated that he
was having, "family trouble" with his brother, and his sister. ARR Blauvelt would not elaborate on what he
meant by, "Family Trouble."
ARR Blauvelt then stated he picked up something from the floor of the apartment, and struck VIC multiple
times. I asked ARR Blauvelt what he used to hit VIC with, and he stated, "something metal." I asked ARR
Blauvelt how many times he struck VIC and he stated, "a bunch of times." I asked ARR Blauvelt why he
did not open the complex gate door when we ordered him and he stated, "I felt bad about what happened." I
asked ARR Blauvelt if he took any drugs or drank alcohol today, and he stated that VIC gave him two
Xanax this morning. ARR Blauvelt did not state anything further regarding the incident. I asked ARR Blauvelt no
further questions.
I sustained a complaint of pain from my right knee from the incident. After I interviewed ARR Blauvelt, Officer
Casillas and I went to Occupational Health for evaluation for our injuries. Officer Casillas and I were evaluated,
and released.

I, Officer G Arias 5520, was working as unit 1C7 in a marked black and white patrol vehicle when I assisted unit
1C68, Officers Casillas #10537 and Cornell #10328, who were responding to regarding a
possible 245 with a brick and the suspect is a male white, 20 to 30 years old with brown shoulder length hair and
not wearing a shirt and the victim was 929 on the balcony for further see call # 1181.
While en route I heard over the radio officers arrive at the location and state that there was a female victim on a
balcony who was down and not responding.I also heard Officer Hodgson ask for LBFD to respond with a ladder
and he also requested for the sergeant to respond with a pick and ram.
Upon arrival I saw that Casillas and Cornell were on scene and I also noticed Officer Hodgson #5611 was heading
to the entry way of the dispatched location carrying a 40mm less-than-lethal unit. I then parked near the entrance
and ran to the entry way of the apartment complex and I could hear one shot from the 40mm from inside the
complex.
As I entered the apartment complex I saw that Officers Cornell and Casillas were on the ground on top of a male
white, later identified as deft Blauvelt, who was on his stomach and refusing to put his hands behind his back. I
could hear the officers telling him to stop resisting multiple times. I went to the lower leg area of Blauvelt were I
saw he was back kicking almost striking Officer Hodgson who was sitting on the back of Blauvelt's hips. I
observed Officer Hodgson deploy his Conductive Electric Weapon (CEW). As there was no room for a proper
spread of the probes he removed the cartridge and used the CEW in a "Drive Stun" mode.
Officer Hodgson was obstructing my view of Officer Casillas and Cornell as I was standing on Blauvelt's ankles
pinning them down to prevent him from further back kicking and possibly striking Officer Hodgson. I was able to
maintain my balance by holding on to Officer Hodgson shoulders. I stood on his ankles for maximum pressure
and because he was kicking back I was not going to put my hands and face near his feet and possibly get struck.
Once I saw that Blauvelt was handcuffed I observed he had blood stains on his back and all over his face that was
later determined to come from his actions prior to the police contact. Officer Cornell told me the suspect had told
him he fell down the stairs and injured himself.
Long Beach Fire was on scene and they rendered aid to Blauvelt and later transported him to Community Hospital
for further see Officer Cornell's Follow up.

Narrative
On 12-30-17 at approximately 0012 hours,my partner (Officer T. Larson #10923) and I (Officer K. Foley #10918)
were working uniformed patrol as Unit 1A78 when we assisted other PD units at Community Hospital (Call
#1181).
Upon arrival I took custody of arrested Kyle Blauvelt who was awaiting treatment in the emergency room
(handcuffed to his bed).
Once Blauvelt was medically cleared by Community Hospital staff, my partner and I escorted Blauvelt to 400 West
Broadway to complete the booking process. Blauvelt was placed in the backseat of our marked black and white
police vehicle with his seatbelt fastened. We then transported Blauvelt to 400 west Broadway.
While at 400 west Broadway, Blauvelt was booked for attempted murder (664/187 (A) PC), assault with a deadly
weapon not a firearm (245 (A)(1) PC), Assault to commit mayhem (220 PC), domestic violence (273.5(A) PC),
battery on a police officer (243(C)(2) PC), resist arrest (69 PC) and resist arrest (148(A)(1) PC). Blauvelt was
escorted to the 6th floor men's jail.
Once we were inside of the men's jail, a strip search was conducted and Blauvelt's pants were collected for
evidence.
Officer T. Larson (#10923) recovered Blauvelt's pants from Jailor M. Rosales (#11058). Officer Larson kept this
item in his possession until placing it into evidence. For further, refer to Property Report.

Narrative
On 12/29/17 at approximately 1732 hours I, Detective Conrad received a call from Officer Williams regarding a
victim with severe facial injuries stemming from a domestic violence incident presently being transported to St
Mary Hospital. Officer Williams further stated the suspect was in custody as well; being transported to Community
Hospital after a use of force with responding patrol units. Officer Williams could not provided me any further
details on the victim's injuries, only stating they were very bad, very significant, and very very bloody.
At approximately 1830 hours I responded to St Mary Hospital, contacting Officer Williams who advised me of the
following:
On 12/29/17 at approximately 1650 hours, Long Beach Police received a called from an off-duty LAPD officer
regarding a male subject repeatedly striking a female subject with a possible brick on the balcony of
The LAPD officer further reported the male subject dragged the now bloodied female subject into the
apartment.
Officer Williams advised me the subject refused to answer/open the door for responding patrol units and the door
was ultimately forced open resulting in a "use of force" involving several officers and the male subject, later
identified as suspect Blauvelt. (For further see UOF follow-ups).
Officer Williams stated as she entered the apartment LB Fire Personal was treating the victim who appeared to be
unconscious and non-responsive. Officer Williams stated the victim was in very bad shape, covered in blood,
with several visible deep lacerations to her entire face. Officer Williams stated there were several broken items
throughout the apartment with blood covering the floors and ceiling. Officer Williams stated several photographs
were taken of the scene.
Officer Williams stated she was unable to determined the weapon used at this point, having walked out onto the
balcony and finding several broken items stained with fresh blood, including a broken clay pot, glass ashtray, and
two white iron objects. Officer Williams stated all items were photographed and collected.
Officer Williams requested and was granted an Emergency Protective Order.
Upon seeing the victim myself, in my opinion, having worked in the domestic violence detail for the last (5) years,
this is possibly the worse facial trauma I have seen. In my opinion the victim's facial injuries resembled that of
driver/passenger involved in an automobile accident that were not wearing their seat belt or an inexperienced
fighter going against an MMA fighter. The victim was unrecognizable against her identification picture.

Narrative
As I introduced myself to the victim as the handling detective I noticed several visible lacerations. The most
significant visible injury was a laceration that began at the top of the victim's lip and ran into her right nostril. I
could see and hear the victim was in extreme pain, therefore I did not ask any questions at the time. I did
however take several photographs and (3) short video clips. (For further see attached CD).
St Mary Hospital Surgeon Doctor, Doctor asked if I would be interested in looking at the victim's CT-Scan.
Doctor showed me several images stopping at one image in particular. The image was of the victim's
skull. Doctor pointed out (2) dark black areas on either side of the victim's nose, stating, you see these
two black areas? I replied, yes. Doctor continued, this entire dark area on both sides of the victim's nose
should be white; these should be the area of her cheekbones. They're gone, they're destroyed, there is nothing
left of them Doctor stated, it's like touching jello at this point.
Doctor continued showing my images stating the victim's nose and jaw line between the bottom of her
nose and upper lip was also broken. Doctor stated in his opinion the victim's injuries were caused by a
blunt, hard object, and not a fist or a punch. I asked Doctor what type of treatment the victim would need.
Doctor stated he had no idea until performing surgery.
I re-entered the victim's room where I noticed Doctor was stitching up the victim's lacerations. After
complication, I asked Doctor approximately how many stitches were used. Doctor stated she lost
count after 60, but would estimate the count at approximately 70 to 80 stitches. Doctor stated the victim
was in very bad shape and would remain hospitalized for several weeks.
On 01/01/18 at approximately 1000 hours I responded back to St Mary Hospital to check the status of the victim,
but was advised she had been transferred to another hospital for further treatment.

Narrative
On 01/02/18 at approximately 1100 hours I responded out to St Francis Hospital in the City of Lynwood to
re-contact the victim. As I entered her room the victim began asking; "Please, please, please tell me he's
(Blauvelt) in jail." I advised the victim suspect Blauvelt currently remains in my custody, but warned he is still able
to be out at this time.
As I looked over the victim's facial injuries I noticed several slight improvements from when I first observed her
back on 12/29/17. A majority of the swelling had gone down, owning the victim to fully open both eyes. I did note
however bruising, which I had not seen back on 12/29/17 possible from the victim's face covered in blood and
petechiae. New photographs were taken of her improvement, along with the fresh injuries.
I asked the victim if she could recall anything from the 12/29/17 incident. The victim stated she was cooking
Carne Asada for dinner, but suspect Blauvelt believed she was cooking him human meat and began yelling at her.
The victim stated all she can remember is suspect Blauvelt punching her one time in the face with a closed right
fist. The next thing she remembers is waking up in the hospital in extreme pain and confused. The victim stated
she has no memory of the events between getting punched in the face and waking up in the hospital.
I asked the victim if suspect Blauvelt has been diagnosed with any metal illnesses. The victim stated suspect
Blauvelt suffers from anxiety and takes medication daily, but has not been diagnosed with any other illnesses.
I asked the victim if there have been any reported or unreported incidents of domestic violence between them.
The victim replied, other than verbal abuse, there hasn't been any other abuse.
Before leaving I speak with St Francis staff regarding future treatment for the victim. Staff advised the victim is
currently waiting to be seen by a surgeon specialist and could not provide any further because they don't know.
I was able to obtain a small medical record from St Francis before leaving. (See attached).
