Narrative
Unit 1C31 (Officer D. Hara #5226) also responded to photograph the scene,and me. We transported
to St. Mary Medical Center in order for him to be medically evaluated and cleared for booking in a jail
facility. While at the hospital,spontaneously stated that he knew that he had done wrong and should
not have run from us. He also stated that he was in the park to meet a friend to give her a speaker cable.
stated that he had smoked crystal meth earlier in the day and had recently started using it. He stated
that he received his disability payment check on December 1st and a female friend introduced him to the drug that
day.stated that he has been smoking meth since then. He stated that it has had a bad effect on him
and that it caused him to black out today.stated that he ran because he blacked out when he saw our
police vehicle and the paranoia from the meth use caused him to run.
was treated by Emergency Room Physician Dr.He was subsequently admitted to the hospital
for his narcotics use and other concerning unrelated medical issues. Sgt. Kuroda was advised of his admittance
and RNB.

Narrative
Officer Carillo later provided me with the following items:
1) Discharged ECD cartridge,
2) Broken glass pipe with what appeared to be a bulb at one end, and was burnt throughout. The pipe is
commonly used to smoke methamphetamine.
3) Clear plastic bindle containing an off-white crystal substance believed to be methamphetamine
4) 3" glass pipe with a wire mesh at one end and burnt throughout (commonly used to smoke rock cocaine).
I kept the items in my possession until placing them into evidence.

Narrative
I, Sergeant R. Kuroda (DID# 5018), was working in uniformed patrol in a marked black and white police vehicle as
Unit 1S3. I responded to Hoffman Avenue to investigate an arrest that involved the use of force.
I contacted Officers G. Carrillo (DID# 6184) and A. Sturgeon (DID# 10055) who briefed me regarding the incident.
For further see their report.
I then spoke to SUS who was seated handcuffed in the rear of a police vehicle. I noticed that he was
sweating profusely but was calm when I spoke to him. The following is a summary of the interview that was
digitally recorded and uploaded into Mideo.
He said he was "Tasered" by the police because he was scared and that he did not cooperate with them. He did
not hear them, he takes , and does not remember running from the police. He was at the bus
bench on Anaheim near our location. He said he was scared of the police due to a history of "police and
beatings."
He went to the rear of Hoffman and he did not know that the police were chasing him because he could not
hear anything said to him and could not see because lights were in his face. He then said he told the officer that
he was going to lie down and was "Tasered." He said prior to that he thought that he did hear the officer and he
was hit 3-4 times in the left arm and was pushed down on his head. I asked him why didn't he cooperate with the
police. He said was scared, that he takes and he has problems. I asked him if he grabbed the
officer's leg. He said that he did not remember that. He said the officer kicked him. I told that he wasn't kicked. He
also believed there were 12 people there.
He said he was "Tasered" a total of three times. He heard the officer tell him to roll onto his stomach but he
couldn't and was "Tasered."
I asked him if he was in the park earlier. At first he denied it, then said he was by the trees waiting to sell a blue
speaker. He then said that he was not in the bathrooms at the park and he had just come from his house. He
admitted that the officers found drugs and a pipe in his sock.
He stated that he felt pain to his left arm and right knee from the incident. Officer Hara took photos of
at St Mary's Hospital ER. I noted the following injuries on him: 1/4'" abrasions to right and left shoulders. Probe
puncture wounds to the top of the left shoulder and above the tattoo on the left side of his chest (pectoral area).
No other bruises were noted on his back, torso, legs or head.
Watch Commander J. Gaynor was notified of the use of force.
